Uncle died naming his sister as beneficiary, sister is on Medicaid in a nursing home. How can she skip herself and name her children as beneficiaries?

Disclaiming Ignored By Medicaid

If a person on Medicaid is named the beneficiary under a will or trust, receipt of the money will usually cause a period of disqualification of the person.

"Disclaiming" the money will be effective to have the money bypass the Medicaid recipient for tax and state law purposes but NOT for Medicaid purposes: under federal law, such a disclaimer is ignored. As such, the money will be treated as if received by the Medicaid recipient and gifted to the person(s) who take the money in default.
